NOTES (broken gates, benches, etc, near the playground should be reported to your supervisor ASAP).
This checklist is due in the playground inspector’s office by the 10th of the following month (for example, January’s checklist is due by February 10th).
Date of weekly check
There is enough fall material, especially at slide exits and under swings. The fall material is raked regularly.
There are no sharp points, corners or edges on the equipment.
There are no hazardous protrusions.
There are no entanglement hazards, such as protruding bolts.
There are no crush or shear points on moving parts.
There are no trip hazards, such as exposed footings, rocks, roots, or other obstacles.
The equipment has no rust, rot, cracks or splinters, especially where it comes in contact with the ground
There are no worn, broken or missing parts on the equipment,especially moving parts.
All equipment is tightly bolted and secure.
There are no loose, worn or missing fasteners, such as bolts, shackles, clamps, chains, and clevises.
The entire play area has satisfactory drainage.
The entire playground is free from debris and litter.
The play equipment is free of insects, spiders, and rodents.
Tree limbs are at least 7 feet away from play equipment.
